Sixth Grade Medical Forms
All incoming 6th grade students must hand in medical paperwork by August 1st. This includes a valid physical submitted on the New Jersey Department of Education Annual Athletic Pre-Participation Physical Examination Form, as well as proof of vaccinations: Tdap (Tetanus, Diptheria, Pertussis) and Meningococcal. Please reach out to our nurse Phyllis Gerber ([email protected]) if you have an appointment that falls after August 1st for these requirements.
Student Athlete Medical Forms (all grades)
Please note that the Annual Pre-Participation Athletic Physical is valid for 365 calendar days. For any students (6th, 7th or 8th grade) trying out for sports teams the physical must be on the Annual Pre-Participation Athletic Physical Form and must be valid on the first day of try-outs. In addition, the following forms as also required for all student/athletes:
- If the physical is older than 90 days then a Health History Update must be completed and signed by the parent.
- A signed State Mandated Athletic Fact Sheet Signoff
- COVID Return to Play (if applicable)
Please be sure to submit these forms by August 1st, as the nurse must review and the district physician must approve each child's paperwork prior to participation in any sports team or try-outs.
We are unable to know ahead of time the district physician's schedule, meaning that turning in forms close to the beginning of the fall sports season does not ensure that the physician will be able to clear your child before the season starts. We understand some physicals may be scheduled for dates after August 1st, but before the start of school. If you fall into this category, please reach out to our nurse, Phyllis Gerber ([email protected]) so you can work together to make sure your child is cleared for try-outs.
If your child has special medical needs such as food allergies, asthma, seizure disorder, need for an epi-pen, etc. you will need to fill out and submit additional forms
