Our spring sports season will begin try-outs in mid-March. In an effort to make sure all students who wish to try out for a spring sport have the opportunity to do so, we are getting a head start on the process. Please use this link to sign up for our Spring Sports Survey by February 21st.
As you know there are certain forms mandated by the State of New Jersey for students to be able to participate in interscholastic sports.
In order for your child to be able to try out for a spring sport, they must have the paperwork below either on file or submitted to the nurse by March 3rd.
These forms can all be found on the HMS Athletic website.
- Annual Athletic Pre-Participation Physical Examination Form
- Health History Update Form
- State Mandated Athletic Fact Sheet Signoff
- Student-Athlete Expectations Form
Once the paperwork has been received, their name will go onto the try-out roster for that specific sport. The coach and/or school nurse will then be in contact with you if any further information is needed.

In order for your child to be able to try out for a spring sport, they must have the paperwork below either on file or submitted to the nurse by March 3rd. The nurse must review and the district physician must approve each child's paperwork prior to participation in any sports team or try-outs. We are unable to know ahead of time the district physician's schedule, meaning that turning in forms close to the beginning of the spring sports season does not ensure that the physician will be able to clear your child before the season starts.
