Necessary Health Forms for Incoming 6th Graders
Physical Examination Form - All student physicals must be handed in on this form.
Students must also provide documentation of the following immunizations:
- All Students entering 6th grade must receive one dose of Tdap (Tetanus, Diptheria, Pertussis).
- All students entering 6th grade must receive one dose of Meningococcal-containing vaccine.
**If your child has special medical needs such as food allergies, asthma, seizure disorder, need for an epi-pen, etc OR your child is interested in trying out for a sport at HMS, you will need to fill out and submit additional forms. These forms can be found on the Health Office Forms webpage.
