Parents of all incoming students must provide the Health Office with the following documents:
- A current medical examination form completed, signed and dated by your child's pediatrician. (see the Health Forms webpage for more information and to download form)
- Incomplete forms will not be accepted! The physical must have been performed within 365 days of the first day of school.
Students
who do not have a complete medical record may be subject to exclusion
from school until all documents have been submitted and reviewed.
