HMS Student Athlete Requirements - All Grades
The HMS Fall Sports Season will begin shortly after the first day of school, signing up on this form is the first step. You must sign up on this form: Fall Sports Sign Up 2026. In addition, there are certain forms mandated by the State of New Jersey for students to be able to participate in interscholastic sports.
In order for your child to be able to try out for a fall sport, in addition to having a current physical examination on file, they must turn in all required forms as a complete packet to Holdrum Middle School as soon as possible. These forms can all be found on the HMS Athletics website.
Once the paperwork has been received, their name will go onto the try-out roster for that specific sport. The coach and/or school nurse will then be in contact with you if any further information is needed.
These forms were due by August 3rd, if you have not yet turned them in, it is extremely important that you submit them to the Holdrum Main Office ASAP. Our nurse and district physician must clear each student upon review of the above-mentioned forms. We are unable to know ahead of time the district physician's schedule, meaning that turning in forms close to the beginning of the fall sports season does not ensure that the physician will be able to clear your child before the season starts. We understand some physicals may be scheduled for dates after August 3rd, but before the start of school. If you fall into this category, please reach out to our nurse, Phyllis Gerber (pgerber@rivervaleschools.com) so you can work together to make sure your child is cleared for try-outs.
Please note that the completion of this form does NOT guarantee that your child will be eligible to play a fall sport. If any of the required forms are not submitted and approved by our district physician before the beginning of the fall season, your child will not be able to participate.
