Necessary Health Forms for Incoming 6th Graders
Required for all incoming students, must be on this form. Pages 1-4 of this document is to be kept on file at your child's doctor's office. Only Page 5 "PPE Medical Eligibility Form" is to be returned to HMS. Physical must have occurred within 365 days of first day of school.

PPE Medical Eligibility Form
This is the only physical exam medical form signed by your doctor that should be handed into HMS. This form is also included in the packet linked above (Page 5) titled: Annual Athletic Pre-Participation Physical Examination Form - Complete Packet.

*Items marked new for this year: we will continue to accept the previously used physical paperwork forms during this transition time, as long as the physical exam is still valid, until the start of the 2024-2025 school year in September.
2. DOCUMENTATION OF IMMUNIZATIONS
Students must also provide documentation of the following immunizations:
- All students entering 6th grade must receive one dose of Tdap (Tetanus, Diptheria, Pertussis).
- All students entering 6th grade must receive one dose of Meningococcal-containing vaccine.
**If your child has special medical needs such as food allergies, asthma, seizure disorder, need for an epi-pen, etc OR your child is interested in trying out for a sport at HMS, you will need to fill out and submit additional forms. These forms can be found on the Health Office Forms webpage.
