HMS Fall Sports tryouts will begin shortly after the first day of school, signing up on the Fall Sports Sign Up Survey is the first step. This must be completed by August 9th. In addition, there are certain forms mandated by the State of New Jersey in order for students to be medically cleared to participate in interscholastic sports.

It is extremely important that these forms are turned in by August 9th. Our nurse and district physician must clear each student upon review of the above-mentioned forms. We are unable to know ahead of time the district physician's schedule, meaning that turning in forms close to the beginning of the fall sports season does not ensure that the physician will be able to clear your child before the season starts. We understand some physicals may be scheduled for dates after August 9th, but before the start of school. If you fall into this category, please reach out to our nurse, Phyllis Gerber ([email protected]) so you can work together to make sure your child is cleared for try-outs.
Please note that the completion of this form does NOT guarantee that your child will be eligible to play a fall sport. If any of the required forms are not submitted and approved by our district physician before the beginning of the fall season, your child will not be able to participate.
